Create a member ​

Role: KPZ administrator.
Where: Member → Create Member (/admin/members/create).

User story: As a KPZ administrator, I want to enter a member organisation and its contacts so that KPZ has a record to manage.

Before you start ​

Search Member for the organisation first to avoid creating a duplicate. Have the organisation's Chamber of Commerce (KvK) number, address, email, work description, contact details, employee count, and WZD status ready. If it belongs to a collaborative organisation, have its membership number or confirmation that the number is pending.

1. Open the form ​

  1. Sign in with a KPZ administrator account.
  2. Choose Member in the sidebar.
  3. Select Create Member.

This is the staff-assisted form. Applicants registering themselves should use the public registration flow.

2. Enter organisation information ​

Organisation emailA valid organisation email address. Wait for the availability check.
Chamber of Commerce numberThe organisation's KvK number. This field is required.
SectorSelect the relevant sector. If you choose Other, describe it.
Short description of your workDescribe the care or support provided and to whom, in no more than 400 characters.
AddressStreet, house number, postal code, city, and country. All are required.
Type of healthcare and Target groupSelect the applicable options. Describe any Other option selected.
Collaborative organisationIf affiliated, enable the affiliation option and select the organisation, or enter its name under Other. Supply a membership number or mark it as pending.

The selected collaborative organisation may require a particular membership-number format. Follow the hint displayed in the form.

3. Add contact persons ​

  1. Enter the primary contact's first name, last name, and email.
  2. Complete salutation, phone, and role where applicable.
  3. Use Add Contact Person for additional contacts and complete their details.
  4. Check the correspondence categories. At least one contact must receive general correspondence and at least one must receive financial correspondence. When there is only one contact, categories are assigned automatically.

The primary-contact help text identifies this person as the account holder: their email is used for sign-in and account communications. Check it carefully; the organisation email and the primary contact email have different purposes.

4. Enter membership details ​

  1. Set the WZD status to match the organisation.
  2. Enter the number of working persons as a whole number from 1 to 1,500.
  3. For more than 1,500 people, follow the custom-quote message instead of reducing the number to fit the form.

5. Create and check the record ​

  1. Review the organisation, contacts, and membership details.
  2. Select Create Member and wait for the result.
  3. On success, the form displays a member-created confirmation and a Back button.
  4. Return to Member, search for the organisation, and open its record.
  5. Check the recorded details and the membership status shown there.

Expected result: A member record has been created. Creation alone does not establish that a registration was approved, a payment completed, or a certificate became available. Check those statuses on the record.

The administrator form does not ask you to draw an applicant's signature. Public registration includes its own signature and terms step.

Common problems ​

Employee-count errorEnter a whole number within the supported range.
Server rejects a fieldCorrect the specific error shown; do not assume the record was saved.
